Effective May 1, 2026. By creating a The Horizon account or using the app, you enter into an agreement with The Formularie LLC ("The Formularie", "we", "us"), and you agree to these Terms. If you do not agree, do not use The Horizon.
Eligibility
You must be 18 years of age or older and legally capable of forming a binding contract in your jurisdiction to use The Horizon. If you create an account on behalf of another person, you represent that you have that person's authorization to do so.
Account responsibility
You are responsible for keeping your account credentials confidential and for every activity that occurs under your account. Notify us promptly if you suspect unauthorized access. We strongly recommend a unique password and, where supported by your device, biometric unlock. The Formularie will never ask for your password by email or chat.
What The Horizon is
The Horizon is a self-tracking and pattern-intelligence app for adults using or considering hormone therapy (HRT for women, TRT for men). The app lets you log symptoms, medications, lab values, body metrics, and journal entries, and surfaces insights — trends, correlations, cycle predictions, daily-state reads — computed locally against your own data using rule-based clinical logic.
What The Horizon is NOT
The Horizon is NOT a healthcare provider. The Horizon is NOT a clinic. The Formularie does NOT employ prescribers, physicians, nurses, or pharmacists. The Horizon does NOT provide telehealth services. The Horizon does NOT diagnose conditions. The Horizon does NOT prescribe medications. The Horizon does NOT issue or refill prescriptions. The reads and insights surfaced in the app are informational and educational only. They are not medical advice, not a substitute for evaluation by a licensed clinician, and not a basis on which to start, stop, or change any therapy. Always consult your own licensed prescriber for diagnosis, prescriptions, and any change in therapy.
Acceptable use
You agree not to:
- Use The Horizon to circumvent applicable laws, including controlled-substance laws.
- Enter another person's health information without that person's authorization.
- Attempt to access accounts, data, or systems that are not yours.
- Reverse-engineer, decompile, or disassemble the app except as expressly permitted by law.
- Interfere with or disrupt the service or the servers that host it.
- Use the service to harass, defame, or harm another person.
- Resell, sublicense, or otherwise commercially exploit access to The Horizon.
License you grant The Formularie
You retain ownership of the data you enter. You grant The Formularie a limited, worldwide, royalty-free license to host, store, transmit, display, and process that data solely to provide the service to you and to perform internal operational tasks (backups, error correction, security investigations). We do NOT acquire any right to sell, rent, or transfer your data to third parties.
License The Formularie grants you
Subject to your compliance with these Terms, The Formularie grants you a personal, non-exclusive, non-transferable, revocable license to install and use the The Horizon mobile app on devices you own or control, solely for your own personal, non-commercial use.
Termination
You may terminate your account at any time from Settings → Delete Account. We may suspend or terminate your account if you violate these Terms or applicable law, or if continued service would expose The Formularie or other users to legal or security risk. Upon termination, your in-app license ends and your per-user data is processed per the Privacy Policy's deletion section.
Disclaimer of warranties
The Horizon is provided "AS IS" and "AS AVAILABLE" without warranties of any kind, whether express, implied, or statutory, including but not limited to warranties of merchantability, fitness for a particular purpose, accuracy of any computed read, non-infringement, or that the service will be uninterrupted, error-free, or secure. Your reliance on any insight, prediction, or read from the app is at your own risk and is no substitute for the advice of your licensed clinician.
Limitation of liability
To the maximum extent permitted by law, The Formularie LLC and its members, officers, employees, and contractors are not liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, or for any loss of profits, revenues, data, or goodwill, arising out of or in connection with your use of The Horizon. The total cumulative liability of The Formularie for any claim arising out of the service is limited to the greater of (a) the amounts you paid The Formularie for the service in the twelve months before the claim arose, or (b) one hundred United States dollars.
Indemnity
You agree to indemnify and hold harmless The Formularie LLC and its members, officers, employees, and contractors against claims, damages, liabilities, costs, and expenses arising out of (i) your violation of these Terms, (ii) your violation of any third-party right, including any health-information or privacy right of another person whose data you entered into the app, or (iii) your misuse of the service.
Governing law and venue
These Terms are governed by the laws of the State of New Mexico, United States, without regard to its conflict-of-laws principles. Any dispute arising out of or related to these Terms or the service will be resolved in the state or federal courts located in New Mexico, and you consent to the personal jurisdiction of those courts — unless applicable consumer-protection law in your jurisdiction requires a different venue.
Changes to these Terms
We may update these Terms as the product evolves. The current version date is shown at the top of this page. Material changes will be surfaced in-app and, where required by law, you will be asked to re-consent before continuing to use the affected feature.
Severability
If any provision of these Terms is held unenforceable, the rest remains in full force, and the unenforceable provision is replaced with the enforceable provision that most closely matches the intent of the original.
Entire agreement
These Terms, together with the Privacy Policy and any consent records you sign at signup, are the entire agreement between you and The Formularie LLC concerning the service, and supersede any prior agreement on the subject.
